Immunization Events at JPS Clinics

Updated shot records must be provided to the school nurse in order to be placed in a class or get schedules in August.   

Texas Department of State Health Services
Texas school immunization requirements are determined by the state legislature and set by the Texas Department of State Health Services, in conjunction with the Texas Education Agency.

Below is the link to school requirements as well as exemption information and how to request an exemption for obtaining an affidavit.

Texas School & Child-Care Facility Immunization Requirements

Immunization Requirements for Students Entering 7th Grade per the Texas Department of State Health Services
:
  • Booster dose of Tdap (tetanus/diphtheria/pertussis) – within last 5 years
  • Meningococcal vaccine – one dose on or after the 11th birthday

In order for students to receive their 7th grade schedule at the beginning of the school year, a current immunization record is due to the school nurse before August 2022.

Due to Coronavirus some offices have limited hours for well visit appointments. Be sure and schedule your appointments now in order to get the required immunizations and submit the updated records before the deadline. Immunizations may be obtained by contacting one of these:

  • Private Physician Offices
  • Any Tarrant County Health Department, by appointment. 817-248-6066

     Call to see if you qualify and to set up an appointment.

Please send a copy of the shot record to your school nurse once your child has received his/her immunizations.

Estimados padres de familia de 6to grado:
Por medio de la presente, les queremos recordar las vacunas que el Departamento de Salud del Estado de Texas requiere para los estudiantes entrando al 7mo grado.
Las vacunas que se requieren son:

  • Una dosis de Tdap (tétano/difteria/tos ferina), dentro de los últimos 5 años.
  • Vacuna meningocócica– una dosis después de haber cumplido 11 años.
    El estado de Texas requiere que todos los estudiantes tengan estas vacunas para que puedan asistir al ciclo escolar.

Para que los estudiantes puedan recibir su horario de 7mo grado al inicio del ciclo escolar, es necesario entregar la cartilla de vacunación actualizada a la enferemera de la escuela antes de agosto del 2020.

Debido a la clausura como resultado del Coronavirus, hay citas limitadas y la mayoría de las clínicas ofrecen citas de chequeos a primera hora de la mañana, por lo que es importante hacer cita ahora para poder cumplir con este requisito de vacunas antes de la fecha límite.

Pueden obtener las vacunas para sus hijos contactando uno de los siguientes:

  • Doctor privado / médico familiar
  • Clínicas escolares de JPS, con cita únicamente. Teléfono 817-702-1100
  • Cualquier Departamento de Salud del Condado de Tarrant, con cita. Teléfono 817-248-6066.  Llame para ver si califica y hacer cita.

Por favor mande una copia de la cartilla de vacunación a la enfermera de la escuela una vez que su hijo haya recibido las vacunas.
